In connection with the registration of your child for the theatre classes and/or theatrical performances to be conducted by or on behalf of The Community Theatre of Greensboro, and in consideration of your enrollment and/or consent of your child in such classes and/or performances, you agree to the following: I/We have been informed fully on the activities to be offered to my child and consent thereto; represent that my child/children are in good health and physically fit and capable of participation in the theatrical programs and performances to be offered by The Community Theatre of Greensboro; acknowledge the risks and hazards of physical injury inherent in theatrical workshops and performances and hereby assume all such risks and hazards. I/We hereby release, waive, and agree not to assert against The Community Theatre of Greensboro, and each of their respective directors, teachers, officers, supervisors, agents, volunteers, or employees (collectively, the “Releasees”) any claim for injury to my child in consequence of or incident to such training, performance or other activities. Specifically, I/We, on behalf of myself and my heirs, executors, administrators, next of kin, successors, or legal representatives forever release, waive, discharge and covenant not to sue the Releasees for any personal injury, death, disability, property damage, property theft or acts and/or omissions of whatsoever kind and nature including, but not limited to, those caused by its negligence or other acts of the Releasees. In case of emergency, if the staff of The Community Theatre of Greensboro is unable to reach me/us by phone, and it is a situation that appears to require immediate emergency medical assistance, I/We hereby give my permission for the staff to contact 911 and secure treatment for my/our child as named in the emergency information. No refunds will be given if you elect not to participate after completing payment and registration. Registration and all program fees are NON-REFUNDABLE. CTG may choose to offer credit (good for one calendar year) to participant(s) in the event that the participant can no longer attend the camp/class on a case by case determination.
